ACE Programme Final Reporting Form
Use this form to send in the results for students who have completed The ACE Programme. You no longer need to send in the students papers. Please send this form and the $52 processing fee and Health Ed Trust will send back the students badge and Certificate.

Use this form to send in the results for students who have completed The ACE Advanced Programme. You no longer need to send in the students papers. Please send this form and the $52 processing fee and Health Ed Trust will send back the students badge and Certificate
ACE Dementia Programme Final Reporting Form
Use this form to send in results for students who have completed the ACE Dementia Programme. You no longer need to send in the students papers. Please send this form and the $52 processing fee. Health Ed Trust will process the Form and send the students badge and certificate back to your facility.

Training Agreement July 2011
All students working on the new ACE Programme need to register by sending in a Training Agreement. Please also send correct ID. See Guidelines to completing the Training Agreement - the next document
If student results arrives at Health Ed Trust and we do not hold a Training Agreement for this the results will be returned to the facility.
- Please send ALL training agreements to Health Ed Trust. Unit 1, 54 Wordsworth St, Sydenham Christchurch 8023.
- If you are unsure if a student has a current training agreement please contact Health Ed Trust.
Training Agreement Guidelines
Use these guidelines to help you complete the Training Agreement
VoC will no longer be required after 25th July 2011
for students who are yet to commence study.

NZQA Number
Every student is allocated an NZQA number when a Training Agreement is sent to Health Ed Trust (if they don't already have one). Having an NZQA number does not mean a student has a current Training Agreement.
Assessor Application Form (PDF)
This form is also part of the On-site Assessors course and needs to be completed to become an Assessor for ACE.
Cross Crediting from the old ACE Core Programme, ACE Dementia Series, & ACE Core Programme Stage 2
Please follow these guidelines if you have students who have completed the old ACE Programmes.
Cross Crediting for students who have completed the old National Certificate in Support of the Older Person - by not using the ACE materials
Do you have a student who has completed the NCSOP using Careerforce or another providers materials? Then refer to this document for the cross crediting to the new ACE Programmes and National Certificates.
