What to Send (PDF 40k)
This one page form lets you know what paperwork for each programme is needed so that when your student papers are sent to Health Ed Trust they can be processed smoothly.
ACE Core Programme Final Checklist (PDF 18k)
When students have completed the ACE Core Programme this checklist needs to be sent to Health Ed Trust with all the students course papers.

ACE Dementia Series Prerequisites Checklist (PDF 104k)
When a R/N or E/N has completed the ACE Dementia Series they must also complete this checklist. R/N’s and E/N’s will be awarded unit standards 5012, 5019 and 5020 on completion of this form. These unit standards are pre-requisites for the ACE Dementia Series – see the next form for a full explanation

Additional Student Information (PDF 48k)
Use this form when you are sending in a student paper for the first time. It is provides important information needed for NZQA, the ITO and the Health Ed Trust database. Only send once for each student.
Training Agreement (PDF 301k)
This is a Careerforce (CSSITO) form and is sent directly to Careerforce. A Training Agreement needs to be completed when a student begins an ACE Course. A current Training agreement needs to be held by Careerforce for any training a student is completing. Without this the students qualifications can not be placed on the NZQA framework.
Please ensure that the Training Agreement:
- has the Health Ed Trust logo in the corner.
- the correct box on page 2 is ticked to relate to the ACE programmes the student intends to complete. For example if the students is only completing the ACE Core Programme tick this box. If the student intends to complete the full NCSOP then the last box is ticked.
- If the student has sent in a Training agreement for a previously completed programme (for example the Core Programme) a new Training agreement will need to be sent in if the student wishes to complete another programme. If you are unsure please call the office.
Health Ed Trust receives a fortnightly list from Careerforce with the names of students who have sent in Training Agreements. If you are unsure if a student has a current training agreement please contact the Trust.
NZQA Number: Once only requirement Every student is required to have an NZQA number. The students qualifications are recorded against this number on the NZQA Framework. There is a $25 once only fee (the hook on fee or registration fee) to be allocated with this number, which is paid directly to Careerforce. Students may have received a number from some other form of study, please check with the student to see if this is the case.
Assessor Application Form
This form is also part of the On-site Assessors course and needs to be completed to become an Assessor for ACE. This form can also be completed if a Registered Nurse, Occuaptional Therapist or Physiotherapist wishes to assess unit standard 5012. |
