ACE Dementia Series Prerequisites Checklist (PDF 104k)
When a R/N or E/N has completed the ACE Dementia Series they must also complete this checklist. R/N’s and E/N’s will be awarded unit standards 5012, 5019 and 5020 on completion of this form. These unit standards are pre-requisites for the ACE Dementia Series – see the next form for a full explanation.
Additional Student Information (PDF 48k)
Use this form when you are sending in a student paper for the first time. It is provides important information needed for NZQA, the ITO and the Health Ed Trust database. Only send once for each student.
Health Ed Trust 2009 Training Agreement (PDF)
Health Ed Trust have signed a Contract with Careerforce on 24th March 2009. All students working on the new ACE Programme need to register by sending in a Training Agreement. Please also send correct ID.
If a student paper arrives at Health Ed Trust and we do not hold a Training Agreement for this the paper will be returned unmarked to the facility.
- Please send ALL training agreements to Health Ed Trust. PO Box 130184, Christchurch 8141.
- The $25 hook on fee was abolished in December 2008.
- If you are unsure if a student has a current training agreement please contact Health Ed Trust.
All students on work permits are required to have this Variation as well as a Work Permit. If students apply for this at the same time as they apply for their Work Permits the $120 fee is waived. Please send a copy of the Training Agreement with your application.l
NZQA Number
Every student is allocated an NZQA number when a Training Agreement is sent to Health Ed Trust (if they don't already have one). Having an NZQA number does not mean a student has a current Training Agreement.
Training Agreements for old ACE Programmes
We are no longer accepting Training Agreements for these three programmes.
Assessor Application Form (PDF)
This form is also part of the On-site Assessors course and needs to be completed to become an Assessor for ACE.
Cross Credit for ACE Dementia Series
(PDF)
If you have students who have completed the ACE Dementia Series then they are eligible for a straight cross credit to the four new Dementia unit standards. Please complete this form and a Training Agreement and send into Health Ed Trust. We will do the rest.
